Authorize to amend an existing contract with NH Hospital Association, Concord, NH, (originally approved by G&C on 5/4/16, item #14), for maintenance and support of the NH Uniform Healthcare Facilities Discharge Data System data collection, by increasing the price limitation by $8,000 from $1,095,916 to $1,103,916 with no change to the contract completion date of September 30, 2025.
