Description
Authorize to enter into an agreement with Nick Perencevich, MD, Concord, NH, to continue to serve as Physician Investigator of the Medical Review Subcommittee for FY16, for an amount not to exceed $20,000. Effective July 1, 2015 or upon G&C approval, whichever is later, through June 30, 2016. 99% General, 1% Impaired Physician Program/Agency Income.
